In early 2003, a campus-wide master plan for Chinle CHCF, based on the approved PJD/POR, was created to address future growth of the facility to the year 2012. Expansion and renovation options for all services were developed along with construction phasing based on priority.
Phase One of the Campus Master Plan expanded and renovated portions of the existing hospital to respond to the most urgent needs in providing additional Ambulatory Care Services. The goal of the Chinle CHCF leadership was the creation of a clinic design that is the manifestation and physical extensions of a patient care team (consists of 4-5 providers and appropriate support and ancillary staff) with empaneled patients. The goal of the facility is to allow the patient care team to provide excellent care for its patients.
An 'Integrated Clinic' concept was created to achieve the goal of a single point care for patients by moving the provider from Primary Care Clinics, Specialty Care Clinics and Mental Health Clinics to the patient as opposed to moving the patient to the providers. The proposed plan eliminates traditional borders between clinics by grouping assigned assets of each clinic to provide staff efficiency within each clinic as well as allowing easy staff access between clinics.
Care coordinators and mental health providers were located within the Primary Care Clinics to help bridge the patients' physical and mental healthcare by providing a whole person approach to patient care in one setting "Integrated Clinic".
